This clinical trial is studying how a personalized program can help improve physical function in elderly individuals who are experiencing frailty. Frailty is a condition that affects many older adults, making them weaker and less able to perform daily activities. In this trial, 160 participants aged 60 and older will be recruited from Beijing Chaoyang Hospital. To qualify, they must be identified as frail according to a specific scale called the Fried scale. Participants will work with healthcare providers to set personal goals for their physical health, and those in the intervention group will receive tailored support to help achieve those goals.
Participants can expect to be followed for 3 months and then again at 24 months to see how well the program works in improving their strength and daily functioning. It’s important to note that individuals with certain conditions, such as severe cognitive impairment or advanced cancer, will not be eligible to participate. If the program proves effective, it could offer valuable insights into managing frailty in older adults, leading to better care in everyday medical practices.

Eligibility criteria
- • The inclusion criteria were the following: (1)≥60-years old; (2) recognized as frailty by Fried scale.
- • The exclusion criteria were the following: (1) considered with life expectancy \< 1 year such as advanced cancer patients; (2) difficult to communicate with such as patients with severe cognitive impairment (Mini Mental State Examination score ≤ 17); (3) severe hearing disord.
