This clinical trial is focused on improving how young people living with HIV transition from pediatric to adult healthcare. It aims to test a new approach, led by youth themselves, to help make sure that these young people get the support they need during this important change. The trial will compare two different strategies at clinics in Kenya: one that uses standard training for healthcare providers and another that includes a youth-led initiative to enhance the transition process. The goal is to see if the new approach helps more young people receive necessary care, improves their health outcomes, and encourages healthcare providers to consistently use the transition support package being tested.
To participate in this study, healthcare providers must be at least 18 years old and work at one of the participating clinics. They need to provide HIV services to adolescents. Young people aged 10-24 who are living with HIV and receiving care at these clinics may also be eligible. Participants can expect to take part in surveys and focus group discussions, and their medical records will be reviewed to track how well the transition support package works over time. This study is not yet recruiting participants, but it aims to gather valuable information that could lead to better care for young people living with HIV.

Eligibility criteria
- Inclusion Criteria:
- • Healthcare providers
- • ≥18 years of age
- • Employed at one of the 32 facilities in the RCT
- • Provides HIV services to adolescents affiliated within their clinic Adolescents and youth living with HIV
- • Between 15-24 years of age (focus group discussions, surveys) or between 10-24 years of age (offered the intervention)
- • Receives HIV clinical care at one of the clinics included in the study
- • Attended clinic ≥1 time following intervention implementation
- Exclusion Criteria:
- Any individual who meets the following criteria will be excluded from participation in this study:
- • - Conditions that would place the individual at increased risk or preclude the individual's full compliance with or completion of the study
