ClinConnect Summary

This clinical trial is studying how Community Health Workers (CHWs) can help patients with uncontrolled diabetes and high blood pressure by addressing social barriers that may affect their health. The trial aims to find out if a program led by these workers can lower blood sugar levels and blood pressure in patients receiving care at community health centers. If you or someone you know has Type 2 diabetes with a recent blood sugar test showing levels of 9% or higher, or has high blood pressure readings of 140/90 mmHg or above, you may be eligible to participate in this study.

Participants in the trial can expect to receive support from a Community Health Worker who will help identify and address social needs that may be impacting their health. The study is currently recruiting individuals aged 18 and older, and it does not have a gender requirement. This research is important because it will help community health centers better implement programs to support patients with similar health challenges in the future.

Eligibility criteria

  • Inclusion Criteria
  • 18 years or older
  • People with Type 2 diabetes with their most recent hemoglobin A1c test result (a blood test that reflects average blood sugar levels over the past 2-3 months) is greater than or equal to 9%.
  • People with essential hypertension with their last systolic blood pressure (BP) (the top number in a BP reading) is greater than or equal to 140 mmHg or diastolic BP (the bottom number in a BP reading) is greater than or equal to 90 mmHg.
  • Exclusion Criteria
  • People who are less than 18 years old
  • Pregnant people
  • People who don't meet the Type 2 diabetes or hypertension criteria.
