This clinical trial is exploring how effective a medication called midodrine is when used alongside norepinephrine to help critically ill patients reduce their reliance on vasopressors, which are drugs that help raise blood pressure. The goal is to see if midodrine can make it easier for patients to stop using these medications after being treated for shock, a serious condition where the body doesn’t get enough blood flow.
To participate in this study, patients need to be between 65 and 74 years old and must have been receiving norepinephrine for at least 24 hours while being stable enough for treatment. However, patients with certain conditions, such as severe heart problems or an allergy to midodrine, will not be eligible. If someone joins the trial, they can expect to be monitored closely as they receive midodrine along with their usual treatment. This research could provide valuable information on improving care for patients in critical situations.

Eligibility criteria
- Inclusion Criteria:
- • Patients with refractory hypotension on an intravenous vasopressor for more than 24 h but deemed clinically stable.
- • Patients on noradrenaline support \<10 μg/min
- Exclusion Criteria:
- • Known allergy to midodrine.
- • Severe shock state, as evidenced by multiple vasopressor infusions or high vasopressor requirement (i.e. noradrenaline \>10 μg/min)
- • Patients with severe organic heart disease (e.g., heart failure, sever aortic stenosis), pregnancy, thyrotoxicosis or pheochromocytoma.
- • Those with no enteral route available.
