This clinical trial is exploring a new way to treat headaches, including migraines and tension headaches, using an injection of 0.5% bupivacaine given in the neck area instead of traditional intravenous (IV) medications that are often used in emergency rooms. Many people go to the emergency department (ED) for headache relief after other treatments have failed. Unfortunately, the medications used in the ED can sometimes cause unwanted side effects and might not always provide the relief that patients need. This trial aims to see if the bupivacaine injection can offer quick and lasting pain relief with fewer side effects compared to the usual IV treatments.
To participate in this study, you must be between the ages of 65 and 74, be able to speak and understand English, and have a non-traumatic headache that causes a significant amount of pain (rated 5 or higher on a scale of 1 to 10). Participants will receive either the bupivacaine injection or the standard IV medications chosen by the doctor. The trial is not yet recruiting participants, but it's designed to help determine the best way to treat headaches safely and effectively in the emergency setting. If you have any concerns or questions about participation, the research team will provide all the necessary information and support.

- Inclusion Criteria:
- • Patient has to be able to provide own consent
- • English-speaking only
- • Non traumatic headache with VAS pain score of 5 or above out of 10
- Exclusion Criteria:
- • traumatic headache
- • Confused patients
- • allergy to bupivacaine
- • infection over injection site
- • recent neck surgery
- • hemodynamically unstable
- • bleeding disorder
- • anticoagulation use
- • pregnant, lactating women
- • prisoners
