This clinical trial is comparing two different surgical methods for treating benign prostatic hyperplasia (BPH), a common condition that causes an enlarged prostate in men. The two methods being studied are called bipolar transurethral resection and enucleation of the prostate. The goal is to see which method works better and is safer for men with medium-sized prostates that are causing symptoms.
To participate in this trial, men aged 45 to 80 years old who have symptoms of BPH (like difficulty urinating) and have not found relief with medication may be eligible. Their prostate size must be between 40 to 80 cubic centimeters, as measured by a specific ultrasound test. However, men with prostate cancer, previous prostate surgeries, or certain other medical conditions will not be able to join. If someone qualifies and agrees to participate, they can expect to undergo one of the two surgical procedures while being monitored for their safety and recovery.

Eligibility criteria
- Inclusion Criteria:
- • Male patients aged 45-80 years with symptomatic BPH (IPSS ≥8).
- • Prostate volume between 40-80 cc as determined by transrectal ultrasound (TRUS).
- • Failure of medical management.
- • Written informed consent.
- Exclusion Criteria:
- • Prostate cancer (suspected on PSA/DRE or confirmed by biopsy).
- • Previous prostate/urethral surgery.
- • Neurogenic bladder or urethral strictures.
- • Significant coagulopathy.
- • Uncontrolled urinary tract infection.
- • Patient on anticoagulant medication.
