This clinical trial is looking at how a special program that uses a continuous glucose monitor (CGM) and remote patient monitoring (RPM) can help people with uncontrolled type 2 diabetes manage their blood sugar levels better. The study will last for one year and aims to see if using these tools can lower average blood sugar levels (measured by A1c) and reduce feelings of stress related to diabetes. Participants will wear a CGM that continuously checks their blood sugar levels, allowing them to see how their levels change with food, exercise, and medication. A healthcare team will monitor this data remotely and reach out to help adjust treatments if necessary.
To be eligible for the trial, you must be at least 18 years old, have type 2 diabetes, and have an A1c level higher than 8%. You cannot participate if you are pregnant, have used a real-time CGM for more than six weeks in the last three months, or have certain serious health conditions. Participants will also fill out a questionnaire about their feelings related to diabetes, which will help the team understand how the program affects both blood sugar control and emotional well-being. Overall, this trial hopes to find better ways to support individuals in managing their diabetes, especially in underserved communities.

- Inclusion Criteria:
- • Adults 18 years or older with Type 2 diabetes AND
- • A1c \> 8%
- Exclusion Criteria:
- • Pregnancy (participant-reported)
- • Previous real time CGM use for more than 6 weeks in the last 3 months
- • Use of steroids
- • Active cancer treatment
- • Terminal illness
- • Dementia, mental impairment
- • Type 1 diabetes
- • End stage renal disease, on dialysis
