ClinConnect Summary

This clinical trial is looking at how the position of patients during a specific kidney stone surgery, called percutaneous nephrolithotomy (PNL), might affect their heart health afterward. The study will compare two positions: lying on the back (supine) and lying on the stomach (prone). Researchers want to see if either position leads to a higher risk of Myocardial Injury after Noncardiac Surgery (MINS), which is when heart damage occurs after surgery, indicated by high levels of a protein called troponin in the blood. About 400 patients aged 40 and older who are scheduled for this surgery will participate, with half in each position.

To be eligible for the trial, patients should not have any serious heart conditions, and they must be willing to give consent to join the study. Participants can expect to have their heart health monitored before and after the surgery through blood tests and heart scans. This research aims to help doctors understand if the way patients are positioned during surgery could lead to safer practices in the future, ultimately protecting heart health after such procedures.

Eligibility criteria

  • Inclusion Criteria:
  • Age ≥40 years.
  • Patients undergoing percutaneous nephrolithotomy (PNL) for kidney stones.
  • Willing and able to provide informed consent.
  • Exclusion Criteria:
  • Known coronary artery disease, heart failure, or other significant cardiovascular conditions.
  • Elevated preoperative troponin levels (≥0.03 ng/mL).
  • Major intraoperative complications (e.g., severe bleeding requiring transfusion).
  • Inability to comply with study procedures.
