This is an observational study at Hospital Universitario San Ignacio in Bogota, Colombia, looking at how well the Minimed 780G Advanced Hybrid Closed Loop insulin system (AHCL) helps adults with Type 1 diabetes manage blood sugar around surgery. Researchers will track glucose data before, during, and after elective or urgent surgery to see how often glucose stays in a target range (100–180 mg/dL) during the operation, and how much time, if any, is spent below safe levels. They’ll also examine a safety measure called THRH (time at higher risk of low blood sugar) and how these factors relate to overall perioperative safety.
Who can join: adults aged 18 and older with Type 1 diabetes who use the AHCL Minimed 780G and are undergoing elective or urgent surgery during the study period. People are not eligible if their insulin pump data aren’t available during the perioperative period, if they are pregnant, or if they don’t have CGM sensor data for the two weeks before and after surgery. The study is still enrolling (about 15 participants planned), and it uses data uploaded from CGM devices to analyze glucose patterns. While safety oversight is in place, results are not yet available, and participants should expect their usual care team to monitor and may have small adjustments to glucose targets around surgery to reduce the chance of lows.

- • Inclusion Criteria:Patients older than 18 years AHCL (Minimed 780G) users undergoing elective and urgent surgery during the study period.
- Exclusion Criteria:
- • Patients whose pump use was discontinued during the perioperative period. Pregnant patients. Patients without sensor data in the 2 weeks before and after surgery.
