This clinical trial is looking at how a six-month program that provides vouchers for fruits and vegetables can help young people who have received a kidney transplant. To be eligible for this trial, participants must be between 2 and 25 years old, have received a kidney transplant, and have recently been identified as facing food insecurity. If the participant is under 18, their caregiver must also be willing to take part. Those who are only receiving nutrition through methods like tube feeding or total parenteral nutrition will not be included. Participants can expect to receive vouchers that they can use to purchase healthy foods, which may help them feel better both physically and emotionally as they adapt to life after their transplant. This study is important because it aims to find new ways to support the health and well-being of young kidney transplant recipients.

Eligibility criteria
- Inclusion Criteria:
- • Age: Children and young adults aged 2 to 25 years who have received a kidney transplant.
- • Food Insecurity: Patients who have screened positive for food insecurity (FI) within the past six months, based on the Hunger Vital Sign
- • Patients who are currently receiving care at the transplant center and whose caregivers (if the patient is less than 18 years of age) are willing to participate in the intervention.
- • Caregivers of non-adult patients
- Exclusion Criteria:
- • Patient being on a diet consisting of only total parenteral nutrition or tube feeds.
