The MATCH-UP trial is studying a new way to provide cancer care to patients with breast or prostate cancer from the comfort of their own homes, using enhanced telehealth technology. However, individuals who are currently part of another clinical trial or speak a language not supported by the hospital's system cannot join. If you are eligible, you can expect to receive personalized care from your doctor remotely, which may include video consultations and other digital tools to help manage your treatment effectively.

- Inclusion Criteria:
- • Patients with breast or prostate cancer at Memorial Sloan Kettering Cancer Center who have had at least 3 prior medical oncology visits.
- Exclusion Criteria:
- • Patient is enrolled on a therapeutic clinical trial.
- • Patients whose primary language is not currently supported by Epic.
