The Health Questionnaire Study is looking to understand how to improve the way people answer questions about their feelings of anxiety and depression. Specifically, the researchers want to see if using certain phrases, like "bothered by," changes how participants respond to a questionnaire. This study is open to adults aged 18 and older who live in the U.S. and have been diagnosed or are receiving treatment for generalized anxiety disorder or major depressive disorder in the past year.
If you decide to participate, you will fill out a questionnaire with some special instructions. This study is currently recruiting participants, so if you meet the eligibility criteria, you may have the chance to contribute to important research that could help improve how these questionnaires are designed for future patients. Your involvement could help make it easier for others to express their feelings and get the support they need.

- Inclusion Criteria:
- • 18 or older
- • U.S. resident
- • Self-report being treated for or receiving a diagnosis generalized anxiety disorder or major depressive disorder by a licensed provider (e.g., MD, DO, LPCC, LCSW, LMFT) in the past 12 months
