This is an observational study from Ankara, Turkey that is checking whether a common neck-and-head posture issue, called forward head posture, is related to how severe fibromyalgia symptoms are. Researchers will compare people aged 18–55 who have fibromyalgia with a control group of people with knee pain but no neck problems. They plan to enroll about 134 people and will measure posture by taking one side-view photo (with permission) to calculate the craniovertebral angle, an easy way to describe how far the head sits forward. The same visit includes completing a Fibromyalgia Impact Questionnaire (FIQ) to rate how fibromyalgia affects daily life.
If you participate, you would have FM diagnosed by standard criteria or be in the control group for comparison. You’ll complete the FIQ, and the researchers will track these measures for up to 12 weeks. Key exclusions include any past or current neck or spine problems, recent neck surgery, major neurological or psychiatric conditions, and certain inflammatory diseases. This study does not test a treatment; it aims to understand whether posture is linked to fibromyalgia severity, which could inform future care. Enrollment is currently ongoing by invitation, led by Nadide Koca, M.D., at Ankara Training and Research Hospital.

Eligibility criteria
- Inclusion Criteria:
- • Aged 18-55 years
- • Diagnosed with Fibromyalgia (FM) according to The American College of Rheumatology (ACR) 2010 Diagnostic Criteria for Fibromyalgia
- Exclusion Criteria:
- • History of past or current pathology in the cervical spine, thoracic spine, or upper extremities
- • History of paraspinal or thoracic surgery involving the head, spine, or shoulders within the last six months
- • Structural spinal malformation
- • History of surgical or neurological disorders
- • Cervical trauma or pain treatment within the last 6 months
- • Temporomandibular surgery
- • Psychiatric disorders (e.g., dementia, amnesia, or delirium)
- • Systemic inflammatory rheumatic diseases
