Respiratory Virus Infections in Acute Heart Failure

This clinical trial is studying how respiratory viruses, like the flu and coronaviruses, might affect patients who are admitted to the hospital with acute heart failure. Researchers will test patients for these viruses within the first three days of their hospital stay and analyze how often these viruses are found in patients with heart failure. They are also interested in seeing if the time of year affects how often these viruses occur and how they change the symptoms of heart failure.
To participate in this study, you need to be an adult aged 19 or older and have been admitted to the cardiology department with signs of acute heart failure, such as a high level of a specific heart-related protein in your blood or symptoms like fever or cough. However, if your heart failure is caused by other clear medical issues, you may not be eligible. If you join the trial, you can expect to have a swab test to check for respiratory viruses, and your health will be monitored closely. This research could help doctors understand the relationship between respiratory infections and heart failure better.

Eligibility criteria
- Inclusion Criteria:
- • Adult aged 19 years or older admitted to the cardiology department with acute heart failure
- • NT-proBNP \>300 pg/mL
- • Clinician judgement of possible respiratory viral infection (fever \>38 degrees or cough, respiratory symptoms such as sputum or consolidation on x-ray or CRP \>5 mg/dL)
- • Patients who have a nasopharyngeal swab for respiratory virus PCR test performed within 72 hours of admission for any of the 3 reasons
- Exclusion Criteria:
- • Patients with a clear other cause of acute heart failure (ex. STEMI, bradycardia requiring a pacemaker, SBP \>200mmHg or DBP \>110mmHg, haemoglobin \<7g/dL)
- • If the cause of pulmonary oedema is prominently attributed to exacerbation of CKD
- • If there is a clear other source of infection that can explain the fever
