This clinical trial is focused on improving care for people with diabetes, especially those at risk for vision problems like diabetic retinopathy (DR). The researchers want to see if using a special patient information system (PIS) can help diabetes care providers work better together when it comes to treating patients with both diabetes and high-risk DR. They will compare two groups of participants: one group will receive regular DR results, while the other will get the same results along with specific care recommendations for managing their diabetes. The goal is to find out how well each approach meets international care guidelines and whether it is cost-effective.
To be eligible for this study, participants must be over 30 years old, diagnosed with diabetes and high-risk DR, and currently being treated at the Dinajpur Diabetes Hospital in Bangladesh. They should also be willing to take part in the study. Those with other major causes of vision loss or medical issues that could make it hard to follow up on care won't be included. Participants can expect to contribute to important research that aims to enhance diabetes care and potentially improve their own health outcomes.

- Inclusion Criteria:
- • Diagnosed with DM
- • Diagnosed with high-risk DR
- • Over 30 years of age
- • Currently treated at the Dinajpur Diabetes Hospital in Bangladesh
- • willingness to participate in the study
- Exclusion Criteria:
- • Other significant causes of vision loss
- • Medical conditions that could impair compliance with follow-up (such as stroke, dementia, etc.)
- • Not willing to participate in the study
