This clinical trial is comparing two treatments for chronic migraines: a procedure called sphenopalatine ganglion block (SPGB) and BOTOX injections. Chronic migraines are defined as having headaches for 15 or more days each month for at least three months, with some of those headaches meeting specific migraine criteria. The goal of this study is to see which treatment is more effective and safe for managing these frequent headaches.

To participate in this trial, individuals must be between 18 and 65 years old and have a history of chronic migraines. However, those with certain conditions, such as headaches caused by medication overuse, bleeding disorders, or allergies to local anesthetics or BOTOX, cannot join. If eligible, participants will receive either the SPGB or BOTOX treatment and will be monitored for their effectiveness and any side effects, helping researchers understand which option may be better for managing chronic migraines.

Eligibility criteria

  • Inclusion Criteria:
  • Age 18 and 65 years.
  • Both sexes.
  • Chronic migraine. Chronic migraine, defined as headaches on ≥15 days per month for ≥3 months, of which ≥8 days meet criteria for migraine without aura or respond to migraine-specific treatment according to International Classification of Headache Disorders-3 Beta.
  • Exclusion Criteria:
  • Patients with medication over use headache.
  • Bleeding disorders.
  • Abnormal neurological examination.
  • History of allergy to local anesthetics or BOTOX.
