This clinical trial is studying how exercise and mental training can help improve the health of older stroke survivors who also have conditions like high blood pressure or type 2 diabetes. The study will take place in community health centers in China, and participants will be assigned to different groups to receive either exercise training, mental training, a combination of both, or standard care for a year, followed by a year of monitoring.

To be eligible for this trial, participants must be at least 60 years old and have had a stroke that is stable. They can also have other health issues, like hypertension or diabetes, but they should be able to participate in a community program for six months. Participants should be able to walk independently to the community center and communicate basic information. It's important to know that this trial is not yet recruiting participants, and those interested should ensure they meet the eligibility criteria before considering joining.

Eligibility criteria

  • Inclusion Criteria:
  • Aged ≥60 years
  • The doctor diagnosed it as a stroke and it is in a stable stage (the diagnosis time is not limited)
  • Patients with concurrent hypertension and/or diabetes may also have other diseases at the same time
  • Be able to participate in a community intervention lasting for six months
  • Be informed and agree to participate in the research
  • Exclusion Criteria:
  • With the aid of a walking aid (not a wheelchair), it is also impossible to walk independently to the community service center.
  • Lack basic communication skills and do not recognize Arabic numerals
  • Suffering from life-threatening diseases, with a survival expectation of less than 6 months, etc.
