This clinical trial is looking at new ways to help patients who have trouble breathing after major surgery. Many patients experience what is called postoperative respiratory failure, which can lead to serious complications and longer hospital stays. Instead of using traditional methods like invasive mechanical ventilation (a machine that helps people breathe), this study will explore two alternative approaches: non-invasive ventilation (NIV) and high-velocity nasal insufflation (HVNI). These methods aim to improve breathing without the need for more invasive procedures.
To be eligible for this trial, participants need to be at least 18 years old and have had major surgery within the last 48 hours. They should also be experiencing difficulty breathing, with a specific measure showing reduced oxygen levels in their blood. Unfortunately, those who are under 18, pregnant, or have certain medical conditions like severe facial injuries or active gastrointestinal bleeding cannot participate. If you join this study, you'll be monitored closely to see how well these breathing support methods work in improving your recovery after surgery.

Eligibility criteria
- Inclusion criteria:
- • Age above 18 years
- • within 48 hours after major surgery
- • PAO2/FIO2 less than 200
- • Signs of respiratory distress and respiratory rate (RR) more than 25 cycles/minute
- Exclusion Criteria:
- • Age below 18 years
- • Pregnant ladies
- • Orofacial trauma or burns
- • Active Gastrointestinal bleeding
