The CORALINE trial is a study looking at how certain medical factors relate to the success or failure of non-invasive respiratory support in patients with acute respiratory failure. This type of support includes treatments like high-flow nasal oxygen and non-invasive ventilation, which help patients breathe without needing a breathing tube. The researchers want to understand when these treatments might not work, which is defined as needing a breathing tube (orotracheal intubation). They will also track outcomes like how long patients stay in the hospital or intensive care unit and whether they survive after 90 days.
To be part of this study, participants need to be at least 18 years old and currently hospitalized in the Respiratory Medicine Unit at AOU Policlinico di Modena. They must have acute respiratory failure, which means their lungs are not getting enough oxygen or are having trouble removing carbon dioxide. Participants will be monitored while receiving non-invasive treatments, and their progress will be recorded over time. It's important to note that individuals who need immediate invasive ventilation, have certain neuromuscular diseases, are pregnant, or cannot provide consent will not be eligible for this trial.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥ 18 years
- • Hospitalization in the Respiratory Medicine Unit at AOU Policlinico di Modena
- • Acute respiratory failure (hypoxemic and/or hypercapnic) with PaO₂/FiO₂ \< 300 mmHg
- • Undergoing treatment with HFNO or NIV
- Exclusion Criteria:
- • Immediate need for invasive ventilation upon admission
- • Known neuromuscular disease
- • Pregnancy
- • Lack of informed consent
