This clinical trial is looking at a new way to help protect the kidneys of patients who are having heart surgery. The main goal is to see if closely monitoring the pressure inside the abdomen and adjusting blood pressure accordingly can reduce the risk of acute kidney injury (AKI) after surgery. Patients will be placed in one of two groups: one will receive regular care with a standard urinary catheter, while the other will have a special catheter that helps track abdominal pressure. By managing blood pressure based on this information, doctors hope to improve kidney blood flow and lower the chances of kidney problems.
To be part of this study, participants need to be adults scheduled for heart surgery that involves using a heart-lung machine, and they must have a specific heart function level (left ventricular ejection fraction below 50%). The trial is not yet recruiting participants, but it aims to gather valuable information on how this new monitoring technique might help keep kidneys healthy during recovery from heart surgery. Throughout their hospital stay, all participants will have routine blood and urine tests to check how their kidneys are doing. This study could lead to better ways to prevent kidney damage in patients after cardiac surgery.

- Inclusion Criteria:
- • Adults Undergoing cardiac surgery with cardiopulmonary bypass
- • Left ventricular ejection fraction (LVEF) \< 50%
- Exclusion Criteria:
- • Current pregnancy
- • ESRD or AKI on dialysis
- • Currently on ECMO or Impella
- • Emergent surgery
