This clinical trial is exploring two methods of anesthesia for elderly patients undergoing hip surgeries, specifically comparing the ultrasound-guided lumbar erector spinae plane block (ESPB) and the pericapsular nerve group block (PENG block). The goal is to determine which method might provide better pain relief during and after surgery. The trial is not yet recruiting participants, but it aims to include men and women aged 65 and older who are scheduled for a unilateral elective total hip replacement surgery and have a body mass index (BMI) between 20 and 35.

To participate in this trial, patients should not have any known allergies to the anesthesia drugs used, nor should they have certain medical conditions, such as psychological disorders or chronic pain. Those who have skin infections at the injection site or blood clotting issues would also be excluded. If eligible, participants can expect to receive one of the two anesthesia techniques to help manage their pain during the surgery, and their experiences will be monitored throughout the process. This study is important as it aims to provide valuable insights into the best ways to manage pain for elderly patients during hip surgery.

Eligibility criteria

  • Inclusion Criteria:
  • Both genders Type of surgery; unilateral elective total hip replacement. Physical status ASA I, II Age more than or equal 65 Years. Body mass index (BMI): \> 20 kg/m2 and \< 35 kg/m2.
  • Exclusion Criteria:
  • Patient refusal. Patients with known sensitivity or contraindication to drugs used in the study (local anesthetics \& opioids).
  • History of psychological disorders and/or chronic pain. Pre- existing peripheral neuropathies Coagulopathy. Infection of the skin at the site of needle puncture area.
