This clinical trial is studying two different approaches to giving blood transfusions to patients who have severe bleeding from their upper gastrointestinal tract (which includes the esophagus, stomach, and the first part of the small intestine). The trial will compare a "liberal" strategy, where more blood may be given, with a "restrictive" strategy, where less blood is used, to see which is more effective and safe for patients.
To participate in this trial, individuals must be between the ages of 18 and 99, have upper gastrointestinal bleeding that has caused anemia (a condition where you have fewer red blood cells), and require a blood transfusion. However, people under 18 or those who have anemia from causes other than upper gastrointestinal bleeding will not be included. If you join this study, you can expect to be monitored closely as researchers assess which transfusion approach works best for your condition. The trial is not yet recruiting participants, so additional details about joining will be available when it starts.

- Inclusion Criteria:
- • all patients presented by upper GIT bleeding and their CBC show anemia and need blood transfusion
- Exclusion Criteria:
- • all patients less than 18 years and patients have anemia and received blood transfusion by another causes other than GIT bleeding
