This clinical trial is studying how a special program called Medication Therapy Management (MTM) can help improve the safety and effectiveness of blood-thinning medications (anticoagulants) for patients who have had surgery for colorectal cancer. These medications are important because they help prevent blood clots, but they can also cause problems like bleeding. The aim is to see if the MTM program can reduce these risks for patients who are at higher chances of complications after their surgery.
To participate in this trial, you need to be between the ages of 65 and 74 and have been diagnosed with colorectal cancer, along with a condition called venous thromboembolism (VTE), which is when blood clots form in the veins. You should also have been on anticoagulant treatment for at least three months. If you join the study, you'll receive close attention and support to help manage your medication and monitor for any side effects. This trial is currently looking for participants, and everyone is welcome regardless of gender. Your involvement could help improve care for future patients facing similar challenges.

Eligibility criteria
- Inclusion Criteria:
- • 1. Patients with histologically confirmed CRC and symptomatic or incidental VTE who received anticoagulant treatment.
- • 2. CRC patients with VTE treated with an anticoagulant for at least 3 moths.
- Exclusion Criteria:
- • 1.Participation in this study required active anticoagulant treatment. Apart from this, there were no specific exclusion criteria.
