This clinical trial is looking at how different types of blood flow during heart surgery might affect brain function after the operation. Specifically, researchers want to know if a pulsating flow of blood is better for preventing cognitive problems, such as confusion or memory issues, compared to a steady flow. The study will measure blood flow in the brain using a special ultrasound technique called transcranial Doppler, focusing on patients undergoing elective heart surgery.
To participate in this trial, you need to be between 18 and 70 years old and scheduled for a specific type of heart surgery that uses cardiopulmonary bypass, which helps circulate blood while the heart is stopped. However, if you have certain health issues, like significant narrowing of the neck arteries or a history of serious stroke, you won't be able to join. If eligible, participants will undergo the surgery as usual but will also have their brain blood flow monitored during the process. This study aims to provide valuable insights that could improve recovery for patients undergoing heart surgery in the future.

Eligibility criteria
- Inclusion Criteria:
- • Patients aged 18-70
- • Patients scheduled for isolated elective cardiopulmonary bypass surgery
- Exclusion Criteria:
- • Patients with carotid stenosis ≥50%
- • Emergency patients or those with a history of reoperation
- • Patients with a history of cerebrovascular events
- • Patients who do not provide consent
- • Patients with temporal window failure
- • Patients with an ejection fraction (EF) \<40%
- • Patients with a Mini-Mental State Examination (MMSE) score \<23
- • Patients who require mechanical ventilation for more than 24 hours or those who are reintubated
