This clinical trial is focused on improving patient safety by addressing medication errors in a hospital setting in Nepal. To participate in the study, patients of all ages who are receiving prescriptions while hospitalized may be eligible. Additionally, healthcare professionals over the age of 18 who work at Dhulikhel Hospital can also take part. Participants can expect to contribute to a significant effort to enhance patient safety by sharing their experiences and insights. It’s important to note that participants whose prescriptions are unclear or incomplete, or those healthcare workers who can’t finish the survey, won’t be included in the results. The trial is still in the planning stage and has not started recruiting participants yet.

Eligibility criteria
- Inclusion Criteria:
- • For the quantitative part, the inclusion criteria are patients of all age groups and prescriptions issued to inpatients.
- • For the qualitative part of the study, inclusion criteria are healthcare professionals aged more than 18 years working in Dhulikhel Hospital.
- Exclusion criteria:
- • The exclusion criteria will be incomplete and unclear prescriptions.
- • The healthcare persons who accepted the consent but couldn't complete the survey will be excluded from the study.
