This clinical trial is focused on finding ways to identify vision problems in older patients, specifically those over 70 years old who have at least one chronic health condition, like diabetes or heart disease. The study aims to see how effective vision screenings are in general practice settings in Denmark. The main goal is to detect any vision impairment, and a secondary goal is to identify eye conditions that may not currently affect vision but still need attention from an eye specialist.
To be eligible for this study, participants must be 70 years or older and have one or more chronic diseases that are being managed by their general practitioner (GP). However, people with dementia, known eye diseases, or those already seeing a private eye doctor cannot participate. If eligible, participants can expect to undergo vision screenings during their regular visits to their GP. This trial is currently recruiting participants, and it seeks to improve early detection of vision issues to help maintain the eye health of older adults.

- Inclusion Criteria:

- • One or more chronic diseases
- • Are followed by GP du to chronic disease
- Exclusion Criteria:
- • Dementia diagnosis
- • Known eye-diseases or are currently followed by private ophtalmologist
- • Not able to understand Danish
