The AMIDA-ICH trial is studying how a team of healthcare professionals, including geriatricians, clinical pharmacists, and nurses, can work together to improve medication management for older adults in an intermediate care hospital. The main goal is to see if this team approach, which focuses on each patient's personal health goals, can help reduce the number of medications older patients take, making their treatment more effective and safer.

To be eligible for this trial, participants should be 65 years or older, have at least three chronic health conditions that require medication, and be able to speak Spanish. During the trial, participants will receive a thorough health assessment and a review of their medications by either the multi-professional team or just a clinical pharmacist. They will also complete some questionnaires about their health. After leaving the hospital, researchers will follow up with participants three months later to check on their health and any potential issues. This trial aims to ensure that older adults receive the best possible care tailored to their needs.

  • Inclusion Criteria:
  • older adults (65+ years)
  • ≥ 3 or more chronic conditions requiring medical therapy
  • able to speak Spanish
  • capacity to provide informed consent or have a surrogate able to consent on their behalf
  • Exclusion Criteria:
  • unable to provide informed consent
  • anticipated length of hospital stay \< 72h
  • estimated life expectancy \< 3 months
  • homeless
  • already enrolled in a drug trial
