Predicting and Monitoring Spontaneous Bacterial Peritonitis in Cirrhotic Patients Using the Mansoura Scoring System

This clinical trial is studying a new way to predict and monitor a serious infection called spontaneous bacterial peritonitis (SBP), which can occur in people with liver disease and fluid buildup in the abdomen (ascites). The goal is to use a simple scoring system, called the Mansoura Scoring System, to identify patients who might have this infection without needing invasive tests. This could help doctors start antibiotic treatment earlier and improve health outcomes.
People who might be eligible for this study are adults over 18 who have liver cirrhosis with fluid in their abdomen and who agree to participate. However, those who recently took antibiotics, are already on preventive treatment for SBP, or have other infections, certain blood diseases, or other health conditions may not qualify. If you join, you will be closely monitored to see if the scoring system can accurately predict SBP, helping doctors decide when to begin treatment quickly and safely. This study has not started recruiting yet but aims to make managing this infection easier and safer for patients in the future.

Eligibility criteria
- Inclusion Criteria:
- • Patients with cirrhosis Presence of ascites Age \> 18 years Provided informed consent
- Exclusion Criteria:
- • Use of antibiotics within the previous two weeks Prophylactic treatment for SBP before admission Ascites not related to portal hypertension (e.g., peritoneal tuberculosis, peritoneal carcinomatosis, congestive heart failure, renal disease, pancreatitis, or hemorrhagic ascites) Secondary peritonitis Infections other than SBP (e.g., pneumonia, urinary tract infection, skin infection) Presence of malignancy Hematologic diseases Use of antiplatelet drugs or non-steroidal anti-inflammatory drugs (NSAIDs) Receipt of platelet or blood transfusion prior to admission Diseases associated with elevated MPV (e.g., diabetes mellitus, cardiovascular disease, hyperthyroidism, immune thrombocytopenia, myeloproliferative disease)
