ClinConnect Summary

This clinical trial is studying whether using a simple video and audio connection between ambulances and remote pediatric emergency doctors can help improve care for seriously ill children during ambulance rides. The study is happening in Karachi, Pakistan, and aims to see if having a doctor guide ambulance staff through a live call leads to better medical decisions and health outcomes compared to the usual care provided by paramedics alone.

Children aged 0 to 14 years who are being transported by ambulance for more than 20 minutes and are in serious condition (based on a priority system used by emergency services) may be eligible to participate, as long as a parent or guardian is present to give permission. During the ambulance ride, some children will receive care guided by a remote doctor through a live video call, while others will receive standard paramedic care. The researchers will collect medical information when the ambulance picks up the child, when they arrive at the hospital, and during hospital triage to compare the two groups. This study could help improve emergency care for children in places with limited resources.

Eligibility criteria

  • CHILDREN
  • Inclusion criteria:
  • i. Age 0-14 years ii. Children transported by an SIEHS ambulance with a transport time of more than 20 minutes iii. Children presenting to ChildLife ED with a parent/ guardian present to consent iv. Children classified as "Charlie, Delta, Echo" on Medical Priority Dispatch System
  • Exclusion Criteria:
  • i. Children transported without an adult parent or guardian
  • EMERGENCY MEDICAL TECHNICIANS (EMTs)
  • Inclusion Criteria:
  • i. EMTs currently employed by the SIEHS EMS service.
  • Exclusion Criteria:
  • i. EMTs who refuse to participate or consent for the study.
  • TELEMEDICINE PHYSICIANS (TMPs)
  • Inclusion Criteria:
  • i. TMPs currently employed by the CLF Telemedicine services.
  • Exclusion Criteria:
  • i. TMPs who refuse to participate or consent for the study.
