This clinical trial is testing a program called My Lung Health Coach (MLHC), designed to help people with COPD (a lung condition that makes breathing difficult) manage their health better. The program is delivered through six one-on-one virtual sessions over 12 weeks with a trained respiratory educator. These sessions cover important topics like understanding COPD, quitting smoking, using medications correctly, staying active, managing mental health, getting vaccinations, handling symptoms, eating well, traveling safely, and planning for the future. Participants will also use a special app linked to their medical records, which they can access on their phone, tablet, or computer to support their learning and self-care.
To join this study, you need to be 40 years or older, have been diagnosed with COPD by a lung specialist, and be a current or former smoker. It’s preferred, but not required, that you have had at least one flare-up of your COPD in the past year. You must be able to give informed consent and willing to follow the study’s procedures, including using the app. People who recently had a flare-up, have asthma, serious other health problems, or cannot use the app on their device won’t be eligible. If you join, you can expect personalized support to better understand and manage your condition from the comfort of your home, which could help improve your quality of life.

Eligibility criteria
- Inclusion Criteria:
- • 1. Must be deemed to have capacity to provide informed consent;
- • 2. Must sign and date the informed consent form;
- • 3. Stated willingness to comply with all study procedures;
- • 4. Be diagnosed with COPD as per a WCH Respirologist
- • 5. Be 40 years or older
- • 6. Be a current or ex-smoker
- • 7. Preferred but not required: have a history of at least 1 COPD exacerbation (requiring prednisone or antibiotic use, or an urgent healthcare visit of any kind) in the previous 12 months
- Exclusion Criteria:
- • 1. Experienced a COPD exacerbation in the 4 previous weeks to study enrollment
- • 2. Co-diagnosis of asthma by a WCH Respirologist
- • 3. Significant co-morbidities that could interfere with program participation or terminal illness
- • 4. Unwilling to download and use the Epic Care Companion app and has a unsuitable mobile device to do this
- • 5. Pregnancy
