This clinical trial is testing a new type of spinal implant called an expandable OLIF cage, used in surgery for certain lower back problems like slipped vertebrae, spinal narrowing, curved spine, or herniated discs. The expandable cage is designed to help restore the space between the bones in your lower back and ease pressure on the nerves. Unlike traditional cages, this new device can be adjusted in size during surgery and allows doctors to add bone material through a special opening to help the bones heal and fuse together better.
People aged 40 to 80 who have one affected level in their lower back and haven’t improved after at least six months of non-surgical treatments may be eligible. If you join the study, you’ll receive either the new expandable cage or the standard cage, and doctors will follow you for a year to see how well you recover. They will check your pain levels, back function, and spinal stability using medical images and your feedback. This study aims to find out if the new cage provides better pain relief, healing, and long-term results compared to the usual treatment.

Eligibility criteria
- • Inclusion Criteria:1: Age between 40 and 80 years (inclusive)
- • 2: Clinically diagnosed with lumbar spondylolisthesis or lumbar spinal stenosis
- • 3: Confirmed single-level lumbar degenerative disease
- • 4: Affected level located at L2/3, L3/4, or L4/5
- • 5: Failure of conservative treatment for at least 6 months
- • Exclusion Criteria:1: Patients requiring direct decompression of the spinal canal
- • 2: History of prior lumbar or retroperitoneal surgery
- • 3: Abnormal anatomy between the iliac vascular sheath and the psoas major muscle
- • 4: Presence of active infection or other significant systemic diseases
- • 5: Inability to complete surgical treatment or comply with follow-up due to various limitations
