This clinical trial is studying a program called Social Prescribing, which aims to help people feel less lonely and improve their overall quality of life by connecting them with social and community services through their doctor’s office. The program, starting in late 2024 in Denmark, involves general practitioners (GPs) referring patients to a special coordinator, called a link worker, who helps connect patients to local support groups, activities, or other social resources. The study will look at whether this approach reduces visits to the doctor, emergency hospital visits, and emergency calls, as well as how it affects patients’ feelings of loneliness and well-being.
Anyone aged 65 to 74 who takes part in this Social Prescribing program will be invited to join the study—there are no other specific requirements or restrictions. Participants can expect to receive support from the link worker to access community services, and their health and social outcomes will be tracked over time to see if the program makes a positive difference. This study is part of a larger research effort to find better ways to connect healthcare with social support, helping people stay healthier and feel more connected.

Eligibility criteria
- • All participants in the SP intervention are invited consecutively to participate in the effectiveness evaluation by the Link Worker. There are no exclusion or inclusion criteria in the effectiveness evaluation of the SP intervention.
