This clinical trial is studying whether a treatment called continuous passive motion (CPM) can help prevent stiffness and muscle loss in the ankle for patients who are very sick and on a breathing machine in the intensive care unit (ICU). CPM involves gently moving the ankle without the patient needing to do anything, which might keep the joint flexible and muscles healthier during long periods of immobilization. The study will compare one ankle receiving CPM treatment to the other ankle that does not, to see if CPM makes a difference.

Adults who are expected to need mechanical ventilation (a breathing machine) for more than five days may be eligible, as long as they do not have certain conditions like nerve or muscle diseases, recent leg surgery or injuries, severe blood flow problems in the legs, or pregnancy. Participants will receive CPM treatment on one ankle twice daily for up to a week or until they leave the ICU, while continuing their usual care. The researchers will use simple measurements and ultrasound scans to check ankle movement and muscle condition before and after the treatment. This study will also look at how safe and practical it is to use CPM in very ill patients.

  • Inclusion Criteria:
  • Eligible participants were adults (≥18 years) with acute respiratory failure expected to require mechanical ventilation for \>5 days.
  • Exclusion Criteria:
  • Neuromuscular disorders, recent lower limb surgery or trauma, critical limb ischemia, amputation, deep vein thrombosis, significant leg wounds, or pregnancy.
