This clinical trial, called the RaDIANT Health Systems Intervention, is studying ways to help people with end-stage kidney disease (ESRD) get started on the process to receive a kidney transplant more easily. The study aims to improve how health systems support patients by addressing barriers that can slow down or block access to transplant evaluations, which are the first step in getting listed for a transplant.
The trial will include adults of all genders who receive care at certain transplant centers and their associated dialysis centers, specifically in Atlanta, SC, Durham, NC, and Indianapolis, IN. If you are treated at one of these centers and have not yet started a transplant evaluation, you might be eligible to participate. Participants will be part of efforts to improve the transplant referral and evaluation process, but the study is not yet recruiting. This research is important because it looks at ways to make it easier and quicker for patients to begin the transplant journey, potentially improving outcomes for people with serious kidney disease.

- Inclusion Criteria:
- • Five large transplant centers (Emory Transplant Center and Piedmont Transplant Center in Atlanta, GA; Medical University of SC; Duke University in Durham, NC; and Indiana University in Indianapolis, IN) \~800 referring dialysis center referring to these transplant centers
- Exclusion Criteria:
- • Individuals referred, initiating evaluation, and waitlisted at non-participating start centers
