This clinical trial is studying how a local anesthetic (a medicine that numbs pain) spreads in the area around the hip after a specific type of nerve block called a PENG block is given. The researchers will use MRI scans, which are special pictures of the inside of the body, to see exactly where the medicine goes. The goal is to better understand why this nerve block works well to reduce pain after hip replacement surgery.
People who might take part are adults (18 years and older) who are scheduled to have a total hip replacement. They need to be in good enough health for surgery and able to safely have an MRI scan before the operation. Participants should not have allergies to local anesthetics, infections where the injection would be given, or certain conditions that make MRI scans unsafe, like some implanted devices or severe anxiety about enclosed spaces. If accepted, participants will receive the usual nerve block before surgery and then have MRI scans to check how the anesthetic spreads. This study is not yet enrolling patients, but it aims to help doctors improve pain relief after hip surgery in the future.

Eligibility criteria
- Inclusion Criteria:
- • Patients scheduled for total hip replacement surgery
- • Age ≥ 18 years
- • ASA physical status classification I-III
- • Signed informed consent obtained
- • Orthopedic indication for preoperative MRI study
- Exclusion Criteria:
- • Allergy to local anesthetics
- • Infection at the planned puncture site
- • Refusal or inability to provide informed consent
- • Age \< 18 years
- • Claustrophobia
- • Altered consciousness or severe dementia
- • Presence of MRI-incompatible implanted devices (e.g., pacemakers implanted before 2000)
- • Pregnancy or breastfeeding
