This clinical trial is studying two different tools used to gently remove front upper teeth (near your smile) when the tooth cannot be saved. The goal is to see which tool—called a piezotome or a periotome—works better at removing the tooth without damaging the surrounding bone and gums. After the tooth is removed carefully, an implant (a small metal post) is placed right away in the empty space to help support a new artificial tooth. This immediate placement can help the new tooth look and work better.
People who might join this study are adults between 65 and 74 years old who need to have a front upper tooth taken out because it’s badly damaged, infected, or loose and cannot be fixed. To be part of the trial, the area where the tooth will be removed must be healthy enough to support an implant right away, without damage to the bone in front of the tooth. Participants should not be heavy smokers or have medical problems that could make healing difficult. If you join, you can expect a careful tooth removal using one of the two tools, followed by placing an implant during the same visit. The study aims to find the best way to protect your smile and support the new tooth with less pain and faster healing.

Eligibility criteria
- Inclusion Criteria:
- 1. Patients of both gender with a non-restorable maxillary anterior tooth in the aesthetic zone indicated for extraction and subsequent immediate implant placement. Including those with:
- • 1. Severely carious tooth that it cannot be restored.
- • 2. Pulpal necrosis or irreversible pulpitis that is not amenable to endodontics.
- • 3. Periodontally involved Tooth with grade 2 mobility or above.
- • 4. Tooth with root caries that can't be restored.
- • 2. Patients with extraction socket Type I, according to Elian's classification
- Exclusion Criteria:
- • 1. Patients with extraction socket Type II or III, with defect in the buccal plate
- • 2. Patients with deciduous teeth.
- • 3. Heavy smokers.
- • 4. Medically compromised patients with conditions that would impede implant osteointegration and healing.
