This study is looking at a new way to help older adults in China who have high blood pressure (also called hypertension) better understand their heart health. During regular doctor visits, patients will receive a simple, personalized report called a “Heart Age” that shows their heart risk based on their recent health check results. The goal is to see if this quick and easy information helps people become more aware of their heart health and encourages them to take better care of themselves, all without making doctor visits longer or more complicated.
To join the study, you need to be 60 years or older, have high blood pressure, have had a government-funded health check within the last three months, and be scheduled for a regular follow-up visit with your doctor. People with severe memory problems, serious illnesses that limit life expectancy, or those already in another clinical study cannot join. If you participate, your doctor will share your personalized Heart Age report during your usual appointment, and you’ll be asked about how this information affects your understanding and management of your heart health. This study is currently looking for volunteers who meet these criteria.

Eligibility criteria
- Inclusion Criteria:
- • Adults aged ≥60 years
- • Diagnosed with hypertension
- • Completed their government-funded annual health check within the past 3 months
- • Scheduled for a routine follow-up consultation within the enrollment window -Able to provide informed consent-
- Exclusion Criteria:
- • Diagnosed dementia or severe cognitive impairment
- • Terminal illness
- • Participation in another clinical intervention trial
