This clinical trial is studying whether using a medicine called dexmedetomidine, given as a mist to breathe in before surgery, can help reduce sudden changes in heart rate and blood pressure during certain steps of weight-loss surgery in very overweight patients. These steps include putting a breathing tube in (intubation) and inflating the abdomen with gas (pneumoperitoneum) to help the surgeon see inside. The researchers also want to see if this medicine lowers the amount of pain medicine (opioids) needed during surgery.
The study is looking for men and women between 20 and 60 years old who are considered morbidly obese, with a body mass index (BMI) between 30 and 45, and who are generally healthy enough for surgery. People with serious liver, kidney, heart, lung problems, uncontrolled high blood pressure, or certain other conditions won’t be able to join. If you take certain medications like opioids or beta-blockers, or have allergies to these drugs, you would also be excluded. Participants will receive the dexmedetomidine mist before surgery and the medical team will monitor their heart and blood pressure closely to see if the medicine helps keep these stable during the procedure. This study is not yet open for enrolling patients.

Eligibility criteria
- Inclusion Criteria:
- • Age from 20 to 60 years old.
- • Both sexes.
- • American Society of Anesthesiologists (ASA) Physical Status I-III.
- • Morbidly obese patients with a body mass index (BMI) between 30 and 45.
- Exclusion Criteria:
- • Patients' refusal
- • Decompansated hepatic, renal, or cardiac disease
- • Expected difficult airway management
- • Uncontrolled hypertension
- • Psychiatric disease
- • Sever pulmonary disease
- • Patients on opioid, alcohol, beta-blockers, or allergy to any of the study drugs.
