Periodontal Regeneration at the Distal Site of Second Molars After the Extraction of Lower Included Third Molars

This clinical trial is studying how well the bone and gum tissues heal around the second molars (the large teeth just in front of the wisdom teeth) after the removal of impacted lower wisdom teeth (third molars). Sometimes, wisdom teeth are stuck or partially stuck under the gums, which can cause problems with the bone and gums near the second molars. After these wisdom teeth are taken out, there can still be damage or slow healing in that area. This study will compare two approaches: a standard extraction alone versus extraction plus the use of a special treatment made from pig tissue (called enamel matrix derivatives) that may help the bone and gums grow back better. Patients will be followed for up to one year with check-ups and X-rays to see how well their mouth heals.
Adults aged 18 to 74 who have impacted or semi-impacted lower wisdom teeth and are healthy (or smoke less than 10 cigarettes a day) may be eligible to join. Participants should have a healthy second molar next to the wisdom tooth and no major dental work or decay on that tooth. If you join, you’ll have the wisdom tooth removed using one of the two methods, and your healing will be checked regularly over the following year. This study aims to find better ways to protect the health of the tooth next to the wisdom tooth after removal, which could help prevent future gum and bone problems.

Eligibility criteria
- Inclusion Criteria:
- • Adult (\< 18 years old) patients with impacted or semi-impacted lower third molars and with indication of extraction
- • Systemically healthy patients
- • Non-smokers or less than 10 cigarettes per day
- Exclusion Criteria:
- • Patients with uncontrolled systemic diseases (ASA III-IV)
- • Medication that potentially affects bone metabolism
- • Absence of adjacent lower second molar
- • Presence of crowns, distal caries or distal fillings of the adjacent second molar
