This clinical trial, called T1CARE, is studying a new way to help young adults with Type 1 Diabetes by addressing social factors that can affect their health, like access to resources and support. The study will compare two groups: one group will get the new support program, and the other will continue with usual care before they have a chance to join the program later. This research is being done at the Yale Adult and Pediatric Diabetes Centers.
To take part, you need to have Type 1 Diabetes, live in the greater New Haven area, speak either English or Spanish, and have at least one social need that affects your health, as identified through a brief questionnaire. Participants in the T1CARE group will work with a Community Health Navigator—a specially trained person who helps connect patients with resources and support through a local organization called Project Access-New Haven. The study is not yet recruiting participants, but if you qualify, you can expect personalized support aimed at improving both your health and overall well-being. People who have difficulty understanding or consenting to the study won’t be able to participate.

- Inclusion Criteria:
- • report at least 1 health-related social need on a screening questionnaire
- • Type 1 Diabetes
- • fluent in Spanish or English who reside in the greater New Haven area
- Exclusion Criteria:
- • cognitive impairment and otherwise unable to give written informed consent about participating in this study
