This clinical trial is studying whether applying a special pain-relief gel containing a medicine called ketorolac directly inside the root canal can help reduce pain after a root canal treatment. Root canal treatment is done to save a tooth when its nerve is infected or dead, but sometimes patients experience pain afterward. This study will compare two types of ketorolac gels—one made with tiny particles called “nano” ketorolac and a regular ketorolac gel—against a placebo gel that doesn’t have any medicine, to see which works best for easing pain.
If you are between 18 and 50 years old, have a single-rooted tooth with a dead nerve and a chronic infection at the tip of the root, and don’t have health problems that affect pain or healing, you might be eligible to join. During the study, you’ll receive standard root canal treatment with local anesthesia to keep you comfortable. After cleaning and shaping the canal, the gel will be placed inside the tooth and left there for about a week before completing the treatment. You won’t know which gel you receive, and neither will the doctors assessing your pain. Your participation is voluntary, and you can leave the study anytime without any problem. The goal is to find better ways to manage pain after root canals and improve patient comfort.

Eligibility criteria
- Inclusion Criteria:
- • Patients aged 18-50 years.
- • Patients who are diagnosed with necrotic pulp and chronic apical periodontitis in a single-rooted tooth.
- • Patients who provide written informed consent.
- • Patients with no systemic diseases affecting pain perception or wound healing.
- Exclusion Criteria:
- • Patients with irreversible pulpitis or acute periapical abscess
- • Patients with a history of NSAID or opioid use within 24 hours before treatment.
- • Pregnant or lactating women.
- • Patients who require more than two-visits root canal treatment due to anatomical or pathological factors.
