This clinical trial is studying whether using a special type of breathing support called low tidal volume (LTV) ventilation during heart bypass surgery (coronary artery bypass grafting, or CABG) helps protect the diaphragm—the main muscle that helps you breathe. During this surgery, patients are usually put on a heart-lung machine, and the lungs may not be actively ventilated (called apnea). This study wants to see if keeping gentle lung movement with LTV ventilation during surgery helps the diaphragm work better afterward and reduces breathing problems like lung collapse, fluid buildup, or pneumonia. The researchers will also check if this method helps patients get off the breathing tube faster and leave the intensive care unit (ICU) sooner.
Adults over 18 years old who are scheduled for planned CABG surgery and are generally in moderate health (classified as ASA class II or III) may be eligible to join. People with certain lung or muscle conditions, emergency surgeries, or those who cannot have diaphragm ultrasound tests will not be included. If you participate, you will be randomly assigned to receive either the special low-volume breathing support or the usual care during surgery. Your diaphragm’s movement will be measured with ultrasound before and after surgery, and the medical team will closely watch your breathing and recovery, including any lung problems, time spent on the breathing machine, and length of ICU stay. The study will take place at Konya City Hospital and aims to include 60 patients.

Eligibility criteria
- Inclusion Criteria:
- • Age over 18 years
- • Male or female patients
- • American Society of Anesthesiologists (ASA) physical status class II or III
- • Scheduled for elective coronary artery bypass graft (CABG) surgery with cardiopulmonary bypass (CPB)
- • Provided written informed consent to participate in the study -
- Exclusion Criteria:
- • History of neuromuscular disease
- • History of thoracic surgery
- • History of pneumothorax or ascites
- • Chronic respiratory failure
- • Pulmonary hypertension or right ventricular failure
- • Emergency surgery
- • Inability to perform diaphragm ultrasonographic measurements
- • Refusal to participate in the study
