This clinical trial is studying a new way to manage pain during a common kidney stone treatment called Extracorporeal Shock Wave Lithotripsy (ESWL). ESWL uses sound waves to break kidney stones into smaller pieces so they can pass more easily. The study is comparing a pain relief method called the erector spinae plane block (a type of local nerve block) to the usual combination of pain medicines given during the procedure. The goal is to see which approach works better to keep patients comfortable.
Adults between 21 and 60 years old who have kidney stones sized 2 to 3 centimeters and are scheduled for ESWL might be eligible to join. To participate, patients need to be generally healthy with a body weight in a normal range (BMI between 18 and 30) and able to give informed consent. People with certain health issues like bleeding problems, infections at the injection site, severe chronic illnesses, or allergies to study drugs cannot join. If you take part, you can expect to receive either the nerve block or the usual pain relief during your kidney stone treatment, and the study team will monitor how well each method controls pain. This research could help improve comfort for future patients undergoing this procedure.

Eligibility criteria
- Inclusion Criteria:
- • Adult Patients undergoing ESWL with 2-3 cm renal stones
- • patients between the ages of 21 and 60,
- • both sexes,
- • BMI ≥18≤30 kg/m²
- • Able to give informed consent.
- • Patients older than 18 years with American Society of Anesthesiologists Physical Status I or II and
- Exclusion Criteria:
- • Coagulopathy or on anticoagulants
- • Infection at the ESPB injection site
- • BMI ≥30 kg/m²
- • Patient refusal.
- • patients with advanced respiratory, renal, hepatic, neurological, or psychiatric disease, pregnant or nursing women.
- • patients with a history of allergies to any study drugs
