This clinical trial is studying a new way to help people manage long-term health conditions like diabetes and high blood pressure using team-based telemedicine visits and remote monitoring from home. This means that instead of always going to the doctor’s office, participants will have virtual visits with their healthcare team and use special devices to track their health from a distance. The goal is to see if this approach makes it easier for people to manage their conditions and improve their overall health.
People who might be eligible for this study are adults 18 years or older who are already part of the Mobile Health and Wellness Program and have conditions such as high blood pressure, diabetes, heart failure, or lung disease. They must be able to speak English, take medications for blood pressure or diabetes, and be able to use the remote monitoring device or participate in phone-based care. However, people with certain types of dementia or without internet access cannot join. Participants can expect to be involved in remote health check-ins and monitoring, helping their healthcare team support them more closely from home. This study is not yet recruiting participants.

Eligibility criteria
- Inclusion Criteria:
- • Participant in the Mobile Health and Wellness Program
- • 18 years old or over
- • Reporting with hypertension, diabetes, congestive heart failure, or chronic obstructive pulmonary disease
- • English-speaking
- • Currently taking blood pressure and/or diabetes medications
- • Capable of engaging with the Health Recovery Solutions (HRS) Remote Patient Monitoring (RPM) device or participating in standard telephone-based care.
- Exclusion Criteria:
- • Prior diagnosis of Dementia; Alzheimer's disease, Lewy body, Parkinson's, or vascular dementia.
- • No access to an internet connection.
