This clinical trial is studying a new way to help people in Nepal control high blood pressure (also called hypertension) using support from community health workers and mobile phone technology. The study will look at whether this approach is easy to use, effective, affordable, and can be continued and expanded in local communities. Health workers will use a mobile app to connect people with high blood pressure to health services and provide lifestyle advice through their phones. The goal is to see if this helps people manage their blood pressure better and stick to their treatment plans.
People who may join the trial are adults between 30 and 70 years old who have been diagnosed with high blood pressure and are already taking medication for it. They must live in the selected communities in Nepal and be willing to participate in the study. Community health volunteers and frontline health workers involved in these areas will also take part by using the mobile app to support patients. Participants can expect to receive help from trained health workers who use mobile technology to guide them in managing their blood pressure and keeping up with their treatments. This study has not started recruiting yet, but it aims to improve health outcomes through a combination of community care and modern technology.

Eligibility criteria
- Inclusion Criteria:
- • Hypertensive patient's should be in the range of 30 to 70 years old living in selected communities with HTN (systolic BP ≥ 130 mmHg or diastolic ≥ 85 mmHg or taking antihypertensive medication)
- • They should have been diagnosed previously and under medication.
- • They should agree to the informed consent
- • FCHVs should be working for the selected communities and agree to the informed consent
- • FLHWs should work for the health facilities in selected communities and agree to the informed consent.
- Exclusion Criteria:
- • Participants who are not able to respond during the data collection period.
- • Participants who are out of municipality during the data collection period.
