This clinical trial is studying whether a medicine called dexmedetomidine, given as a nasal spray before cataract surgery, can help keep blood pressure steady in older adults who have high blood pressure. Cataract surgery is common in elderly patients, but those with high blood pressure can sometimes experience changes in their blood pressure during the procedure, which can be risky. Dexmedetomidine is known to help calm patients and stabilize blood pressure, and this study wants to see if using it this way makes the surgery safer and more comfortable.

The study will include people who are 65 years or older, have stage 2 high blood pressure (meaning their blood pressure is higher than 140/90), and are scheduled for cataract surgery with local anesthesia. People with certain heart problems, allergies to the medicine, or serious liver, kidney, or brain diseases won’t be able to join. Participants will be randomly given either the dexmedetomidine nasal spray or a placebo (a harmless spray without medicine) about 30 minutes before surgery. Doctors will then monitor their blood pressure, heart rate, and oxygen levels during and after the surgery, and also check how satisfied patients and medical staff feel about the experience. The goal is to find out if this nasal spray can help reduce complications and improve overall outcomes for elderly patients with high blood pressure having cataract surgery.

Eligibility criteria

  • Inclusion Criteria:
  • 1. Patients aged ≥65 years.
  • 2. Patients who will have stage 2 hypertension as per ACC/AHA guidelines (Systolic BP \> 140 and Diastolic BP\> 90 mmHg)
  • 3. Elective cataract surgery under local anesthesia.
  • 4. American Society of Anesthesiologists (ASA) physical status II or III.
  • Exclusion Criteria:
  • 1. Allergy or contraindication to dexmedetomidine.
  • 2. Significant baseline bradycardia (\<50 bpm) or arrhythmias.
  • 3. Use of sedative or anxiolytic medications.
  • 4. History of severe hepatic, renal, or cerebrovascular diseases.
