This clinical trial is studying whether temporarily lifting the ovaries and attaching them to the abdominal wall during laparoscopic surgery for endometriosis can help prevent the formation of painful scar tissue called adhesions. Adhesions often occur after endometriosis surgery and can cause discomfort or complications. This study compares patients who receive this temporary ovarian suspension with those who have standard surgery without it, to see if the technique reduces adhesions and improves recovery.
Women with moderate to severe endometriosis who have suspected adhesions near the ovaries or surrounding areas, based on tests before surgery, may be eligible to join. Participants will have laparoscopic surgery and then either have their ovaries temporarily lifted or not, depending on which group they are assigned to. After surgery, they will complete daily questionnaires about pain and how they feel about removing stitches. Experts who do not know which treatment was used will check for adhesions using ultrasound scans at 6 and 13 weeks after surgery. This study does not use any other anti-adhesion products, to focus clearly on the effect of ovarian suspension. If you are considering surgery for more serious endometriosis and want to help advance research on ways to reduce complications, this trial may be a good option.

Eligibility criteria
- Inclusion Criteria:
- • Patients with moderate to severe endometriosis with suspected adhesions, lesions in the ovary, ovarian fossa, utero-sacral ligaments, pouch of douglas, or involvement of the intestine as seen in preoperative assessment who are willing to participate in the study including postoperative follow-up assessments.
- Exclusion Criteria:
- • Patients who are not intended to have an endometriosis operation. Patients with minimal to mild endometriosis and no suspected adhesions or ovarian, ovarian fossa, utero-sacral ligament, pouch of douglas involvement on preoperative assessment.
