This clinical trial is studying a surgical treatment called Joshi's step with buccal mucosal graft urethroplasty, which is used to fix a severe narrowing (called a stricture) in a part of the urethra known as the bulbar urethra. This narrowing can make it hard to pass urine. The surgery uses tissue from the inside of the cheek (buccal mucosa) to help rebuild the narrowed area and improve urine flow. The study aims to see how well this surgery works and how patients recover afterward.
Men between the ages of 65 and 74 who have a very tight but short narrowing in the bulbar urethra caused by non-injury-related reasons might be eligible to join. To qualify, the narrowest part of the stricture must be less than 1.5 cm long. People with strictures caused by injury, previous surgeries in the same area, or longer or completely blocked strictures are not eligible. If you participate, you can expect to undergo this specific surgery and have your recovery and urinary function carefully monitored to understand the benefits and any risks involved. This trial is currently looking for participants who meet these criteria.

Eligibility criteria
- Inclusion Criteria:
- • Patients with primary bulbar urethral stricture.
- • Patients with non-traumatic bulbar urethral stricture.
- • Patients with nearly obliterated bulbar urethral stricture, with the narrowest portion of the stricture is less than 1.5 cm in length.
- Exclusion Criteria:
- • Patients with traumatic bulbar urethral stricture.
- • Patients with redo bulbar urethral stricture.
- • Patients with obliterated bulbar urethral stricture.
- • Patients with nearly obliterated long bulbar urethral stricture.
