This clinical trial is studying ways to better control high blood pressure in people who have recently had a stroke. High blood pressure is a major cause of having another stroke and can affect brain health. The study will compare two approaches: one where people monitor their blood pressure at home, and another where people do this at home plus attend group medical visits to learn more about managing blood pressure together. The goal is to see if combining home monitoring with group support helps lower blood pressure more effectively after a stroke.
People who might join this study are adults who have had a mild to moderate stroke and have high blood pressure, can read and speak English, have internet access, and already have a primary care doctor at Massachusetts General Hospital. Participants will be asked to check their blood pressure at home, and some will also join group sessions about managing blood pressure. The study is not yet recruiting, but if eligible, participants will help researchers find better ways to protect brain health and reduce the chance of another stroke.

Eligibility criteria
- Inclusion Criteria:
- • Incident, mild to moderate, ischemic or hemorrhagic stroke (NIHSS≤15, ICH score≤3)
- • history or new diagnosis of hypertension
- • ability to read and speak English
- • internet access
- • established primary care physician at MGH
- Exclusion Criteria:
- • severe stroke (NIHSS\>15, ICH score\>3)
- • discharge location to inpatient rehabilitation or skilled nursing facility
- • dementia
- • subjects with moderate to severe aphasia without a caregiver to consent on their behalf
