This clinical trial is studying how the number of rotary files used during root canal treatment affects the success of cleaning a specific small canal (called the MB2 canal) in upper first molars, and how it impacts pain after the procedure. Rotary files are small tools dentists use to shape and clean inside the tooth’s canals. The study compares using one, two, three, or four files to see which approach works best and causes the least pain. Patients will be asked to rate their pain on days 1, 2, 3, and 7 after treatment to help the researchers understand how the different file systems affect recovery.
Adults between 20 and 65 years old who need their upper first molar treated for a dead nerve and mild infection, but do not have severe pain or swelling, might be eligible to join. To participate, patients must have a specific canal (MB2) present in their tooth and be generally healthy. During the study, participants will receive standard root canal treatment using one of the file systems, and the dentist will carefully record how well the files reach the canal’s full length. Follow-up visits will be important to track pain and healing. This trial helps dentists find the best way to treat these tricky canals while keeping patients as comfortable as possible afterward.

Eligibility criteria
- Inclusion Criteria:
- • Systemically healthy individuals aged between 20 and 65 years
- • Requiring primary endodontic treatment of maxillary first molars
- • Diagnosis of asymptomatic apical periodontitis and pulp necrosis based on clinical and radiographic findings
- • Presence of a second mesiobuccal (MB2) canal in the maxillary first molar
- • Absence of or only minimal preoperative pain or symptoms
- • Periapical lesions smaller than 5 mm in diameter
- • Ability to comply with follow-up visits and effectively communicate during the treatment process
- Exclusion Criteria:
- • Presence of symptomatic apical periodontitis or acute apical abscess
- • Retreatment cases (teeth previously treated endodontically)
- • Use of medications such as narcotics, antibiotics, sedatives, or antidepressants within one week prior to treatment
- • Presence of a sinus tract, periapical abscess, or facial cellulitis
- • Pregnancy or breastfeeding
- • Inability to understand or follow the study instructions
